We provide boards and gear to kids who may not otherwise have access. Our goal is to remove barriers and create opportunities through board sports. This program helps kids get started, stay active, and be part of something positive.

• In consideration of participation in the Boardroom Program and receipt or use of any equipment provided (including but not limited to skateboards, skimboards, surfboards, snowboards, and related gear), I hereby agree that I, on behalf of myself and/or my child, as well as my heirs, guardians, legal representatives, and assigns, will make no claim against, sue, attach the property of, or prosecute Apex Riders Foundation, Inc., or any of its officers, directors, agents, employees, contractors, volunteers, or affiliates for any injury, loss, or damage arising from participation in the program or use of any equipment provided, whether caused by negligence or otherwise.
• Apex Riders Foundation aims to support as many participants as possible by providing access to boards and gear whenever available. The Boardroom Program is offered on a first-come, first-served basis for boards, equipment, and other in-kind donations. If items are not available at the time of application, participants may be placed on a waiting list. Submission of this form does not guarantee receipt of equipment, and availability may vary, though we always aim to help where we can.
• I have carefully read this agreement and fully understand and acknowledge its content. I understand that this is a release of liability and a contract between myself, Apex Riders Foundation, Inc., and its affiliates.
